Repair toilet leakage

There is a leak in the pipe that connects the water cistern to the toilet bowl. I can see cracks on the black rubber cap on the pipe/connection (it is an old type of toilet) and the water comes out when the toilet is flushed. Need somebody to either repair it or replace the parts, if required.
