Extend small existing slab and add 50mm height to existing slab.

There is an existing slab measuring 1940 mm x 1620 mm. I would like it extended on one end to make it 1940 mm x 2020 mm, plus extend another side in triangular shape of 0 mm to 200 mm over 2020 mm length. The existing slab also needs to be raised by 50 mm. This is in preparation for spa so needs to be level. One side is against house, another has clear access, the third where extension required has reasonable access and last side where triangular extension required is against colourbond fence but that can be removed easily if required. I have calculated less than 0.5 cubic meters concrete required. Site is prepared except for the removal of a few pavers.
