Repair/service automatic garage door - chain slipping

B&D panel/sectional door with Dominator automatic opener - chain has become loose and constantly slipping out of the teethed wheel of the opener and sometimes getting stuck. Using door manually right now. The achor to the wall (just above the door) is bent towards the opener instead of being flush with the wall. so am guessing thats whats causing the slack in the chain - also suspect springs need re-tensioning as the whole thing got very noisy lately. Please provide quote for repair and/or complete service of the garage door. Urgent job. Thanks heaps.
