Concrete driveway crack repair and resurfacing

I have a 4m x 5m concrete driveway section with a few relatively large cracks caused by an old tree root. (to be chopped out soon for new driveway extension).
This has raised the slab in some areas upto 1/3 inch high at the site of the crack. I would like to repair the cracks by grinding and filling with epoxy and then resurfacing the entire driveway, approx 20m in length with a new colour sealer that is similar to asphalt colour.
This is in preparation for a new asphalt driveway that will meet up to the existing driveway after this repair has been completed.
