Quote then Repair of burnt out rangehood motor

The fan on the rangehood has stopped working. I spoke with my insurance company and they said that repairs are covered but first we need a quote with report stating what's wrong.
So we need a quote (they only require one) and then to get the repairs done. I'm fairly certain the motor has burnt out and so we will require a new motor plus have it installed.
The appliance is a Beaumatic rangehood.
